ABSTRAK :

Early detection in newborn with hearing impairment is important, because hearing impairment habilitation can do early to prevent communication delay. Late detection cause speech and language delay with psychological and cognitive ability permanently. There are two variety methods for detection hearing impairment, subjective and objective methods. Behavioral observational  audiometry is an subjective method but the result cannot be trust for newborn hearing screening. Three objective methods are Otoacoustic Emissions (OAE), tympanometer and Auditory Brainstem Response (ABR). Combination of objective methods give maximal detection but not effective and efficient. Length of ABR in hearing impairment detection more complete than OAE and tympanometer but need more long time for examination, expensive price and patient have to be sleep condition. OAE more effective and efficient for universal hearing screening although cannot detect retrocochlear until brainstem impairment. Most cause hearing impairment is cochlear which is spesific target for OAE examination (especially outer hair cell).

Aim of this study to got data outer hair cell cochlear function with Transient evoked Otoacoustic Emissions (TEOAE) examination and correlation in high risk newborn after discharge from Intermediate ward Pediatric Department Dr. Soetomo general hospital.

This research is an analytic observational study used cross sectional design. Population were high risk newborn after discharge from Intermediate ward Pediatric Department Dr. Soetomo general hospital Surabaya. The sampling technique was done consecutive sampling. During study periode (March until July 2009) got 22 sample of newborn after discharge from Intermediate ward Pediatric Department which suitable criteria of this study.

Newborn with low birth weight and low birth weight accompanied¬† hiperbilirubinemia on first rank each 27,27%, followed by low birth weight accompanied septic 22,72%. TEOAE examination result normal outer hair cell cochlear function 20 (90,90%) newborn and outer hair cell cochlear impairment 2 (9,09%) newborn. The high risk factor of septic gave 1 (4,55%) newborn with bilateral outer hair cell cochlear impairment. The risk factor of low birth weight accompanied septic and asphyxia gave 1 (4,55%) newborn with unilateral outer hair cell cochlear impairment. Statistical analysis was performed logistic regression. There was p = 0,998 for high risk factor of asphyxia, septic and low birth weight, while p = 0,999 for high risk factor of hyperbilirubinaemia. All risk factor of was considered not significant because p ‚Č• 0,05.

Conclusion of this reseach, newborn with low birth weight and low birth weight accompanied hiperbilirubinemia on first rank each 27,27%, followed by low birth weight accompanied septic 22,72%. TEOAE examination result normal outer hair cell cochlear function 90,90% and outer hair cell cochlear impairment 9,09%. The high risk factor of septic gave 4,55% with bilateral outer hair cell cochlear impairment. The risk factor of low birth weight accompanied septic and asphyxia gave 4,55% with unilateral outer hair cell cochlear impairment. Asphyxia, septic, low birth weight or hyperbilirubinaemia did not have correlation with outer hair cell cochlear impairment on newborn after discharge from Intermediate ward Pediatric Department Dr. Soetomo general hospital.
